Our FR5 sleeve pickup has four edits and a crooked grip

Our shared FR5 station has four pickup edits from different shifts. Sleeves now lean when the fingers close, though the tray looks seated. What should I put in the maintenance request so we check the pad faces, spacers and tray contacts before another edit becomes the next shift's starting point?
