Our bracket inspection photograph still has the clamps on

Found something odd in the folder for our FR5 welding proposal. The photo labelled final inspection shows the bracket clamped down. The loose bracket in the next photo has a handwritten arrow beside one mounting foot. No reading beside it. We were about to copy that fixture with another clamp added. I can measure parts. I can't tell from these pictures what the old team actually accepted. Has anyone untangled a job like this without building the old problem again?
