@WillBrooks0835 It wasn't fully seated. The locator reduced visible movement, but it didn't stop us leaving the tray short of its seat.

Report both findings. Ask the responsible person to review a clear seating check, then include normal tray replacement in a supervised comparison before calling the handling fixed.

@SofiaBarnes0543 My separate fixture trial improved one condition without resolving changeover. Describing the remaining failure directly communicated more than forcing the whole result into a success-or-failure label.

You could call the locator fixed and leave seating as an operator issue. The mechanism did reduce movement.

We kept the tool and program unchanged for this trial. That makes the locator comparison cleaner, although it's still only a few supervised runs.

If those stayed the same, doesn't that prove the locator helped? I'm not sure why the sample size changes that bit.

I've records of successful replacements alongside the miss. They suggest inconsistent understanding of the seating process, although the reason for that hasn't been established.

The observations support investigating the seating process without settling its cause. Visibility and access may warrant checking alongside the instructions, rather than assuming wording is the only issue.
