Preserve the sequence and the conditions you actually recorded. Which tray was used before and after replacement, whether it was seated against the locator, and what the miss looked like. Unknown details stay unknown; the later miss does not erase the earlier observation, but it limits what you can claim from it.

Our spacer pickups looked steadier after I added a tray locator. A few supervised runs, then a miss after tray replacement. I can't call that a fix; what should the report preserve?
