First tray stayed put after the locator change. Replacement missed a spacer pickup. Improvement, not closure.

Did the replacement move during the check, or was its starting seat different? You can describe the first tray's improvement without assuming the replacement miss has the same cause.

Front edge met the stops. Did not check underneath before the miss.

Have maintenance compare the actual support contacts on both identified trays. A front edge against the stop does not tell you whether the rest of the tray is sitting on the intended supports.

Can the person who changed the tray join that comparison? They may remember something your front view never showed.

Found a folded traveller under the replacement tray. Operator left the paperwork with its batch.

Where were they supposed to put it? If the instruction says keep it with the tray and gives it no home, this isn't just somebody being careless.

We used to hand people a loose setup sheet and then complain when it turned up wherever they could put it. A visible place for the paper was more effective than another reminder to be careful. Ask the operator what they need to read during the change.

Was that traveller present when maintenance compared the seating, or found nearby afterwards? The report should distinguish an observed obstruction from a possible explanation.

Present under the support, Harish. Maintenance documented the lifted corner. Nora, our instruction gave the paper no other location.

Once the seating issue is corrected, include placement and release in the return check. The earlier pickup edits also need their reasons reviewed against the restored setup, not carried forward automatically.

Give the traveller a holder the operator can reach. Then ask them to do the change, rather than admire the new holder.

Holder fitted at the changeover point. Controls reviewed the edits. Original and replacement trays passed the supervised transfer comparison.

Did the operator use the revised paper arrangement during those tray changes? That is the part I would want in the handover, as well as the successful transfers.

Yes. Operator changed all four checked trays, traveller in its holder each time. Seating and complete transfers passed.
