Start at the strip-down. Tool seating and finger contacts, isolated for inspection. What was removed?
another FR5 point nudge isn't fixing this crooked sleeve

Contact pads and the tray stop came off. Nobody wrote down which pad went back on which finger. They look symmetrical from the front.
16 pointsDid the first bad pickup happen immediately after that cleaning?
18 pointsRick, first shift after cleaning. That isn't a precise first occurrence, but it's the earliest one in the handover notes.
9 pointsShow the fitter the back of each pad against the drawing, not just the nice shiny face; a different shoulder or recess is easy to miss when you're cleaning a pile of bits
21 pointsI would check the tray stop before taking the fingers apart again. It moved in the same strip-down, and you can compare its position with the setup drawing without buying anything.
4 pointsCheck both. Tray position won't explain every crooked grip, and neither will pad identity.
13 pointsAre these sleeves from the same incoming batch as the earlier good run?
0 pointsYes, Callum, same incoming batch. Luis, the pad backs aren't the same: one has a deeper locating recess. Maintenance is checking the assembly drawing before putting them back.
9 pointsUse the full assembly drawing. Different backs may be intentional. Find which pad belongs on which finger before anybody decides that making them identical would help.
16 pointsAny answer on the tray stop yet? Two things came off, so I'd still like to hear about the second one.
16 pointsAnd who signs the setup back in? Include the point edits in that review. Correct hardware plus yesterday's compensation could give you another poor pickup.
17 pointsLiam Baker, stop is back at its documented position. Nathan, drawing says the pads have been swapped. Thanks Luis for the back-face suggestion; front-on they really do look like twins.
8 pointsGood to hear the stop was checked. My preference was the quickest inspection first, not leaving the fingers alone once you found something there.
9 pointsSetter and maintenance have restored the pad arrangement and reviewed the accumulated edits. Supervised checks with identified sleeves look square again. We're leaving the job under observation through another changeover before calling it settled.
12 pointsGive the cleaner a clear assembly photo with the distinguishing backs visible. A front view would preserve exactly the confusion you just found.
17 pointsUseful result. Did the strip-down instruction get updated too?
23 pointsLaura, let the person who does the cleaning try following that draft before you approve it; easier to find the missing detail there than at the next pickup
