Treat physical transfer evidence and the saved index as separate parts of the sequence. Review the interruption between placement and saving, with tray, pocket and attempt identity retained. If the outcome cannot be reconciled, hold it for the defined recovery process instead of offering the old destination again. Test a further restart while it is held too.
